Position Description

Pride Health is hiring a Nurse Practitioner in Osceola County, FL! (Contract)

Travel between multiple Long Term Care Facilities in Osceola County

Schedule: Monday-Friday (8am-5pm)

Contract: 7-8 month contract

Pay Rate: $66-69/hour

Specific Duties:

  • Provide specialized care to high-risk health plan members
  • Work as part of an interdisciplinary team
  • Conduct comprehensive assessments
  • Manage acute and chronic medical and behavioral conditions
  • Ensure accurate documentation, including ICD-10 coding
  • Communicate and collaborate with the care team
  • Facilitate advanced illness and care planning discussions
  • Offer counseling and education to patients and caregivers

Perks:

  • No holidays required

Must have qualifications/experience:

  • Nurse Practitioner or Physician Assistant
  • NP Certified in one of the following (FNP, ANP, GNP)
  • Access to transportation
  • 1+ year of experience (LTC, SNF, Geriatric preferred)
